Contact Us Elements widgets

Contact Us Elements widgets are available on any page and allow you to configure a collection of fields to gather information from site users. 

To create a contact form, add the Contact Form widget to your page, then add additional widgets to the form by clicking the + within the widget.

Available Topics

Use this widget to create a drop-down list of potential topics about which your site users may want to contact you. 

Label – Enter the header that displays above your drop-down list of topics. This field is set to Topics by default.

Hint Text – Enter text that shows before the user opens the drop-down list of topics. This field is set to Select Topic by default.

Topics – Click Add to add a topic choice to the drop-down list. Enter a topic, then click Done. You can click Edit to revise a topic or the trash icon to delete it. To create additional topics, click Add. If you have multiple topics, you can drag to reorder them.

Required Field – Check this box if you would like to require users to select a topic when contacting you.

Contact Form

This widget allows you to define site feedback recipients and configure what site users see when they submit feedback. You should create and configure this widget first, followed by the additional Contact Us fields. Click the + on the Contact Form widget to add additional fields to the form.

Email Recipients – Enter the email address or addresses to which you would like site user feedback sent. Separate multiple addresses with commas. Optimizely recommends using a distribution list address in this field. 

Submit Button Text – Enter the text you would like to show on the button a site user clicks to submit their feedback. This field is set to Send by default.

Submit Success Message – Enter the message you would like to display after a site user has submitted their contact form in the rich text box. This message displays in a pop-up window. Default message: Thank you for your inquiry. A customer service representative will contact you shortly.

Email Address

Use this widget to add a field to your contact form to collect site user email addresses.

Label – Enter the label you would like to appear above the email address entry field. This label is required and is set to Email Address by default.

Hint Text – Enter text that shows before the user enters their email address. This field is required and is set to Enter your email address by default.

Required Field – Check this box if you would like to require users to share an email address when contacting you. This box is checked by default.

First Name

Use this widget to add a field to your contact form to collect site user first names.

Label – Enter the label you would like to appear above the first name entry field. This label is required and is set to First Name by default.

Hint Text – Enter text that shows before the user enters their first name. This field is required and is set to Enter First Name by default.

Required Field – Check this box if you would like to require users to share their first name when contacting you. 

Last Name

Use this widget to add a field to your contact form to collect site user last names.

Label – Enter the label you would like to appear above the last name entry field. This label is required and is set to Last Name by default.

Hint Text – Enter text that shows before the user enters their last name. This field is required and is set to Enter Last Name by default.

Required Field – Check this box if you would like to require users to share their last name when contacting you. 

Message

Use this widget to add a field to your contact form to collect site user messages.

Label – Enter the label you would like to appear above the message entry field. This label is required and is set to Message by default.

Hint Text – Enter text that shows before the user enters their message. This field is required and is set to How can I help you? by default.

Required Field – Check this box if you would like to require users to share a message when contacting you. This box is checked by default.

Phone Number

Use this widget to add a field to your contact form to collect site phone numbers.

Label – Enter the label you would like to appear above the phone number entry field. This label is required and is set to Phone Number by default.

Hint Text – Enter text that shows before the user enters their phone number. This field is required and is set to (###) ###-#### by default.

Required Field – Check this box if you would like to require users to share a phone number when contacting you.
