Required steps for Application Insights telemetry on CMS 12 DxP sites

Description

Optimizely DxP sites use Application Insights for telemetry. If it is missing, please ensure you have done the below. 

Steps
Follow steps 4 and 5 from this document. 
Create a CMS site and deploy (optimizely.com)
After deploying, you should see the following in the sources view of your browser's developer tools. 
You should also see requests in the network tab with the hostname js.monitor.azure.com and dc.services.visualstudio.com.
